Improving the health and wellness of our community

September 3, 2015 // SAMC

The Community Benefit Department is pleased to announce the expansion of its Stanford-based Chronic Disease Self-Management program A Healthier You.
August 2015 marked the launch of expansion plans for the program, made possible by the training of 19 volunteers. These volunteers are referred to as “Lay Leaders” and have completed an intense four-day training session to become fully certified to facilitate the program for community members. Volunteers include Fresno State graduate students, medical professionals, and local community members. The addition of these volunteers allows A Healthier You classes to be offered more frequently – in both English and Spanish – and to serve more people.

In the last 7 months, 142 community members successfully completed the six-week program and have seen measurable improvements in their health:

  • 14% increase in self-efficacy
  • 20% increase in cognitive symptom management
  • 45% increase in physical activity participation
  • 27% decrease in health distress
  • 5% reduction in fatigue level
  • 16% reduction in pain level

This program is available for anyone with, or affected by, one or multiple chronic conditions. For more information about the program or to refer someone who you think could benefit, please call Mariana Ramirez (559) 450-3770 or email Mariana.Ramirez@samc.com.
